New Patient Forms and Information
Please download, print, and fill out all of the of the applicable forms below and bring them with you to your appointment. All forms in the “New Patient Form Bundle” are required for new patients.
We have a trained staff available to talk with you and address your concerns about insurance, appointments, directions, clinic hours, procedures, wound care, treatment instructions and more. Please note the following policies:
We accept most medical insurances, but not all. We highly recommend that you confirm your coverage prior to your appointment.
You will be asked to accept responsibility for payment for insurance non-covered services and deductibles. All co-pays are required at the time of office visit.
We very much prefer to avoid collection agency procedures. So we ask that you not ignore our payment-due notifications, and please call and ask about alternative payment arrangements in times of adverse situations.
Appointments are scheduled on a first-come, first-served basis. We strive to work with your availability, but are not always able to do so. Please arrive on time for your appointment. If you arrive late for an appointment, you may be asked to wait until on-time appointees are seen first.
All injectables require payment in full at the time of service. Some injectables may also require a non-refundable deposit.
When you call the office, listen to the instructions for leaving prescription refill requests. If you have not been seen in our office recently, you may be asked to come in for a brief appointment. We want to see you to ensure the prescription is still the most prudent course of action. We know it can be an inconvenience to come in — especially if you’ve been on the medicine for a long time. But competent patient care is our number one priority and the only way to give that is to meet briefly to ensure the medication still makes sense, is still the most effective, and is not causing any side effects. (Click here to read “Why your doctor won’t refill prescriptions over the phone” – a short article that sums up our thoughts on this issue fairly well. ) Prescriptions are refilled during normal business hours only. We do not refill medications on holidays, weekends or after regular business hours.
